About patient sheets

Use a patient sheet to record treatments that you give to a patient and monitor a patient's vitals. A patient can have more than one patient sheet. Other staff can see what treatments the patient is on, and the treatment tasks that are scheduled. When the patient sheet is completed, Vet Radar syncs the information to the patient's clinical record in ezyVet.

Patient sheet types

Vet Radar has two types of patient sheets. This table shows information about the two types of patient sheets:

AnesthesiaUse the anesthesia sheet to monitor a patient during surgery. The anesthesia sheet has an anesthesia chart that Vet Radar uses to show the patient's vitals. You can make many different anesthesia sheets for a patient.
HospitalUse a hospital sheet to record information about a patient's clinical care. You can make many different hospital sheets for a patient.

Patient sheet settings

This table shows information about the settings that you can set in a patient sheet:

Sheet nameThe name that identifies the patient sheet.
Attending doctor (billing)The attending veterinarian. The attending veterinarian is the veterinarian that gives clinical care to the patient at this time.
Attending vet techThe attending veterinary technician. The veterinary technician is the attending veterinarian that gives clinical care to the patient.
Care teamThe Care team. The care team are the veterinarians and the veterinary technicians that gave clinical care to the patient. For example, you can use Care team to show veterinarians or veterinary technicians that the patient had before.
Attending departmentThe department of a related treatment. ezyVet uses the setting to record the applicable department of treatments for invoicing.
Note: For example, a nurse checks in a patient to the emergency department. Vet Radar shows the treatments that staff give to a patient in the emergency department. Subsequently, if the patient must go to the radiology department for an X-ray, Vet Radar shows that you gave the X-ray in the radiology department. The client's invoice shows the treatments of the emergency department and the treatments of the X-ray department.
Integration consultation idThe number of the patient's clinical record in ezyVet. If you select the number that is in the Integration consultation id box, Vet Radar opens the patient's clinical record in ezyVet.
Case ownerThe primary veterinarian that gives clinical care to the patient. If you change the attending veterinarian in Vet Radar, the primary veterinarian stays the same.
Note: For example, the case owner is the veterinarian who starts the patient's clinical care in the primary hospital. If the patient must have an X-ray, staff change the attending veterinarian to the veterinarian who gives the patient the X-ray treatment.
Consultation departmentThe department that staff select in ezyVet when they check in the patient.
Hide discontinued treatmentIf you enable this setting, Vet Radar does not show treatments that staff stopped.

About completed patient sheets in ezyVet

When you complete a patient sheet, Vet Radar syncs the patient sheet to ezyVet. Vet Radar:
  1. Makes a PDF document of the patient sheet
  2. Puts it in the Attachment tab of the patient's clinical record
