Remote consent forms for appointments

You can send a remote consent form when you make an appointment for a client. A remote consent form shows in a client’s web browser. The client enters an electronic signature in the form. As a result, they agree to the applicable fees for the appointment. Because the form is electronic, it is not necessary for the client to go to a physical office to enter a signature.

A remote consent form shows in a client’s web browser. Because the form is online, it is not necessary for the client to go to your organization to enter a signature.

A remote consent form has a related {AppointmentConsentFormLink} template variable. A practice manager adds the {AppointmentConsentFormLink} template variable to an applicable email template. When a client receives an email that uses the email template, the template variable shows a link to the remote consent form.

To use a remote consent form, a practice manager must make a related document template. The document template:
