ezyVet can automatically send a remote consent form when you schedule an appointment for a client.

A remote consent form shows in a client’s web browser. The client enters an electronic signature in the form to accept agree applicable fees and conditions for the appointment.

This table gives information about each step to configure and use ezyVet for remote consent forms:

Step	Information
1	A practice manager makes these settings in an applicable appointment type: They set Send confirmation to YES They set Includes a consent form to YES They use the Consent form template box to select an applicable document template that has the consent form content (for example, fees and other conditions that the client must accept)
2	The practice manager opens the configuration settings for the document template and they make these changes: They select Use in-person signature capture They add one of these template variables to the template content: #WEBFORM-SIGNATURE#[large] #WEBFORM-SIGNATURE#[medium] #WEBFORM-SIGNATURE#[small]
3	The practice manager: Makes a email template or an SMS template as necessary Makes sure that the template has the {AppointmentConsentFormLink} template variable
4	The practice manager configures ezyVet to use the email template or SMS template for appointment confirmation messages.
5	Staff use the appointment type to schedule a client appointment. As a result, ezyVet sends an appointment confirmation message that includes a link to the consent form.
6	The client opens and completes the consent form.
