When staff print a consent form for euthanasia or for a clinical procedure, ezyVet automatically selects a document template to use for the printed form. The template controls the formatting of the printed output. A practice manager can set the document templates that ezyVet automatically uses for the forms.

1. Select Admin.
   
2. In the settings list, select Templates.
   
3. Select Template Settings.
   
4. In the Euthanasia Consent Form box, select the applicable template.
5. In the Procedure Consent Form box, select the applicable template.
6. Select Save.
   
   ezyVet shows a confirmation message:
