The Claim Summary report
The Claim Summary report makes it easy to do reporting for insurance claims.
Use the Claim Summary report to:
- Get an easy view of all insurance claims with data that includes the patient, insurance company, and policyholder
- Make reconciliation easier with one view of benefit data, payment statuses, and policy IDs
- Make it easier to prepare insurance data for audit and compliance
Output data of the Claim Summary report
This table gives each column name of report output and the information shown:

| Claim Reference Number | The reference ID number of a claim. |
| Claim Insurance Number | The insurance number of a claim. |
| Animal Policy Number | The policy number for the patient. |
| Claim Creation Date | The date that a staff member made the claim in ezyVet. |
| Claim Status | Shows the claim status (for example, Claim is Settled or Claim is being Processed). |
| Is Sent | Shows Yes if a staff member sent the claim from ezyVet. |
| Is Integrated | Shows Yes if the claim is from an insurance integration. |
| Claim Supplier Business Name | The insurance company name. |
| Policy Holder Contact Code | The policyholder code. |
| Policy Holder Name | The policyholder name. |
| Policy Holder Business Name | The business name of the policyholder. |
| Animal Code | The patient code. |
| Animal Name | The patient name. |
| Overall Amount | The total dollar value of the claim. |
| Benefit Amount | The dollar value approved for the benefit. |
| Benefit Payment Codes | The payment reference codes. |
| Benefit Payment Status | The payment status (for example, accepted). |
