Configure an appointment type to use a remote consent form

A practice manager can configure an appointment type to use a remote consent form. As a result, staff can select and use the remote consent form for appointments that use the appointment type.

  1. Select Admin.

  2. In the settings list, select Settings.

  3. Select Appointment Types.

  4. Select the applicable appointment type.
    ezyVet shows the settings of the appointment type.
  5. Go to the Properties section.

  6. Set Send Confirmation to YES.

  7. Set With a Consent Form to YES.

  8. In the Consent Form Template box, select the applicable document template.

  9. Select Save.
