Configure an appointment type to use a remote consent form
A practice manager can configure an appointment type to use a remote consent form. As a result, staff can select and use the remote consent form for appointments that use the appointment type.
- Make sure that your ezyVet site uses an e-booking configuration
- Make a document template for the remote consent form that has the
#WEBFORM-SIGNATURE#template variable - Make an email template that has the
{AppointmentConsentFormLink}template variable
- Select Admin.
- In the settings list, select Settings.
- Select Appointment Types.
- Go to the Properties section.
- Set Send Confirmation to
YES.
- Set With a Consent Form to YES.
- In the Consent Form Template box, select the applicable document template.
- Select Save.
