Configure an appointment type to use a remote consent form

A practice manager can configure an appointment type to use a remote consent form. As a result, staff can select and use the remote consent form for appointments that use the appointment type.

  1. Make sure that your ezyVet site uses an e-booking configuration
  2. Make a document template for the remote consent form that has the #WEBFORM-SIGNATURE# template variable
  3. Make an email template that has the {AppointmentConsentFormLink} template variable
To configure the appointment type, do this procedure.
  1. Select Admin.
    The Admin tab
  2. In the settings list, select Settings.

  3. Select Appointment Types.

  4. Select the applicable appointment type.
    ezyVet shows the settings of the appointment type.
  5. Go to the Properties section.

  6. Set Send Confirmation to YES.

  7. Set With a Consent Form to YES.

  8. In the Consent Form Template box, select the applicable document template.

  9. Select Save.

Appointments of the appointment type can use a remote consent form.
