Configure requisition forms for diagnostic tests of New Zealand Veterinary Pathology

To use New Zealand Veterinary Pathology for diagnostic tests, a practice manager must configure the diagnostic requisition forms.

  • Make sure that your ezyVet site has a dedicated supplier contact record for the New Zealand Veterinary Pathology integration
  • Make sure that your site has a configured integration for New Zealand Veterinary Pathology
To configure a requisition form, do this procedure.
  1. Select Admin.
    The Admin tab
  2. In the settings list, select Clinical.

  3. Select Sites Diagnostic Forms.

    ezyVet shows a screen that you can use to add a diagnostic test form.
  4. In the Supplier box, select the applicable supplier contact record.

  5. In the Diagnostic Form box, select the applicable requisition form.

    Note: If the Diagnostic Form box does not show the applicable requisition form, send an email to ezyVet customer support. Make sure that you include a PDF version of the requisition form. And make sure that it does not have patient data and client data.
  6. Select Save.
    The Save button
    ezyVet shows a confirmation message:

ezyVet has a configured requisition form for New Zealand Veterinary Pathology diagnostic tests.
Make a relation between the diagnostic requisition form and the applicable diagnostic test of New Zealand Veterinary Pathology.
