A practice manager can use AI to make a form with an AI prompt or from a PDF document. Then, they can configure an appointment type to use the form. Applicable clients use the Vello client portal to open and and complete the form.

1. Open Vello from ezyVet.
2. In the left sidebar, select Forms.
   

   Note: If the left sidebar is closed, select the forms button:
   
   
   Vello shows a Forms screen.
3. Select Create New.
   
   Vello shows the form designer.
4. Select Generate with AI or Import PDF as necessary.
   Note: Select Generate with AI to use an AI prompt to make the form. After you select Generate with AI, enter an AI prompt. For example, enter Create a Consent Form for a Medical Procedure. Include fields for patient name, date of birth, and signature. Make signature field required.

   Select Import PDF to use a PDF document to make the form. ERROR - unresolved reference (vello-about-vello-forms-1)

5. In the Form Name box, enter a name.
   Note: Form Name does not show to clients.
6. Select the image button, then find and select an applicable image to add to the form (for example, your practice logo).
   
   Note: The form shows the added image above the form title.
7. Continue to add more questions and make changes until the form is prepared.
   Tip: To make sure the Vello keeps your changes, select Save Draft:
   
   

   Tip: To make sure that form will show correctly for clients, select Preview:
   
   

   After you select Preview, select the laptop button to do a check of your form content with different device types:
   
   
8. Select Publish, and in the pop-up window that Vello shows, select Publish again.
   Vello shows the Forms screen. The screen:

   • Has a new entry for the form that you made
   • Shows a status of Published for the new form
