Make a form

After a practice manager makes a form, they can configure an appointment type to use it. Then, applicable clients use the Vello client portal to open and and complete the form.

  • Make sure that you know the settings and functions of the form designer
  • Make sure that you know about form input types and form variables
To make a form, do this procedure.
  1. Open Vello from ezyVet.
  2. In the left sidebar, select Forms.

    Note: If the left sidebar is closed, select the forms button:

  3. Select Create New.

  4. In the Form Name box, enter a name.
    Note: Form Name does not show to clients.
  5. Select the image button, then find and select an applicable image to add to the form (for example, your practice logo).

  6. Select the Add Question button, or drag an input type or a form variable from left sidebar to the button.

  7. Continue to add more questions and make changes until the form is prepared.
    Tip: To make sure the Vello keeps your changes, select Save Draft:

    Tip: To make sure that form will show correctly for clients, select Preview:

    After you select Preview, select the laptop button to do a check of your form content with different device types:

  8. Select Publish, and in the pop-up window that Vello shows, select Publish again.
    Vello shows the Forms screen. The screen:
    • Has a new entry for the form that you made
    • Shows a status of Published for the new form
Vello has a form that is prepared for clients to use.
Configure an applicable appointment reminder to use the form.
