Settings of the Vet Conditions section of an e-claim record

When you make an e-claim, you must enter important clinical information about the patient. The insurance company examines the clinical information that you enter. The Vet Conditions section of the e-claim has the boxes and settings that you use to enter the information.

The table that follows gives information about the settings and boxes of the Vet Conditions section.
Note: Some of the boxes show information that you cannot change directly. This is because the information shown is from other source records (for example, the patient record). If necessary, use the source records to change the information.
Box or sectionFunction
Diagnosis or Signs (*)Enter the presenting problem. The patient has more than one presenting problem, put a comma between each entry.
Note: If the clinical record has a selection in the Presenting Problems box, ezyVet shows the selection in the Diagnosis or Signs (*) box:
Relation between the Presenting Problems box and the Diagnosis or Signs (*) box
Consultation NotesEnter information about the clinical problems of the patient.
Note: If the clinical record has text in the Clinical Description box, ezyVet shows the text in the Consultation Notes box:

RemarksUse this box to enter general information.
Death Or EuthanasiaSelect if the e-claim is related to death or euthanasia of the patient. If you select Death Or Euthanasia, ezyVet shows a Put Down By Recommendation checkbox and a Euthanasia Reason box. Use the setting and box to give information about why death or euthanasia was necessary.
Treatment DatesUse the From box to enter when the clinical record was made. Use the To box enter when clinical record was most recently changed.
Seen Before RelatedIf the patient was seen for a related health problem:
  • You can enter applicable information in the Description box
  • You can enter the date range of the health problem in the From box and To box
House VisitSelect if it was necessary to go to the patient’s residence. You can enter information about why it was necessary in the House Visit Reason box.
Out of HoursSelect if the clinical work was not in business hours. You can enter information about why the work was necessary in the Out of Hours Reason box.
